Complete surveys in Verily Me to provide your health and research data. As part of your research or care program, you may be asked the same survey questions multiple times.
If you’re part of a care program, the information you provide in a survey may be used to support your individualized care.
If you’re participating in a research study, completing a survey provides data for that study.
When a survey is made available to you, it will appear as a task card on your home page. You can submit responses in various formats, including short answers or multiple-choice selections. If there is a due date listed, be sure to complete the survey by the deadline. Completed or expired surveys will be automatically removed from your homepage.

I already started a survey, but I need to come back and finish it later. | If you start a survey and need to finish later, at the top right of the survey, simply click the exit icon (X). Your responses will be saved and the survey will remain as a task on your homepage with a status of “In Progress”. To return to the survey at a later time, click the survey task card and continue to answer survey questions. |
I already submitted a survey, but I would like to change my answer(s). | If you have already submitted a survey, you cannot edit or update your survey answers now. |
